Position: Assistant Program Director - Community Services
Reports to: Program Director II
Conard House, Inc. is a nonprofit organization empowering people and restoring hope. We work with a diverse adult population and strive for diversity by respecting experience, promoting inclusion and building community. Conard House Inc. contracts with the San Francisco Department of Public Health and Department of Human Services to provide a cohesive array of support services to a client population. This position is with Conard House, Supportive Housing Department.
QUALIFICATIONS:
· Bachelor’s degree in a human sciences or social service field. Relevant experience providing direct services to adults with psychiatric disabilities may substitute for the degree requirement.
· Minimum of one year of experience working with homeless or formerly homeless populations, including supervisory or leadership responsibilities.
· Experience with direct service provision to clients with psychiatric disabilities, substance use issues, and/or homelessness.
· Knowledge of Social Security entitlements and the appeals process.
· Computer literacy; familiarity with case management databases (e.g., Panoramic) and money management software.
· Strong advocacy, organizational, and communication skills.
· Ability to work effectively both independently and as part of a collaborative team.
· Commitment to confidentiality, ethical practice, and adherence to HIPAA and agency policies regarding PHI.
· Must be willing to travel around SOMA and Tenderloin neighborhood to support clients.
· Ability to pass a criminal background check.
Experience:
· Assist in planning, developing, updating, and implementing admission and discharge procedures.
· Support the development and updating of procedures for Case Manager charting and Rep-Payee paperwork.
· Help ensure programmatic changes are consistent with agency and program goals and philosophy. Review any changes with the Associate Director of Operations.
· Assist in conducting the annual Client Satisfaction Survey and include data summary results in required reports.
· Stay informed about social issues and political climate surrounding homelessness, mental health, and substance use issues.
· Attend relevant training seminars and conferences on psychiatric disabilities, substance use issues, and homelessness as approved by the Associate Director of Operations.
· Help develop and maintain liaisons with homeless advocacy groups, community organizations, and other service providers.
· Maintain contact with Social Security personnel to ensure efficient and timely processing of paperwork.
· Attend collaborative meetings with contracting agencies as necessary to ensure contractual agreements are met.
· Encourage interactive peer processing at the program, functioning as both facilitator and participant. Assist in scheduling staff retreats as needed.
· Follow agency/HIPAA guidelines for confidentiality and release of information. Ensure files and confidential records are appropriately secured.
· Provide crisis support to Case Managers. Liaise with Police Department and Psychiatric Emergency Services to ensure client and staff safety and appropriate level of care.
· Assist in preparing and submitting weekly, monthly, quarterly, and annual statistical reports.
· Ensure staff follows guidelines for confidentiality and release of information regarding discussion of client cases with outside providers.
· Assist in purchasing or authorizing purchase of supplies and equipment of less than $300. Submit purchase orders and receipts to Yooz.
· Submit personal requests for paid time off and personal time.
· Provide temporary coverage at other program sites as needed.
· Evaluate and monitor cleanliness and safety.
· Familiarize with agency and program health and safety procedures, review policies and manuals, and attend relevant trainings.
· Ensure interactions with clients and staff demonstrate respect for cultural, racial, ethnic, and gender differences. Report any concerns or incidents immediately.
· Ensure staff adherence to the Money Management and Social Security procedure binders.
· Assist in managing and utilizing TrueLink financial services to support clients’ money management needs.
· Participate in Social Security Audits and report outcomes.
· Complete all other duties as assigned by the supervisor.
