  DEPARTMENT: Bridges Supports Coordination Program SUPERVISOR: Director, Supports Coordination                                   Gesher Human Services is a bridge to hope and opportunity for people at work, at home, and in  the community. Gesher’s workforce development, behavioral health, and inclusion programming serves all Metro Detroiters while meeting the needs of the Jewish community.      GENERAL JOB DESCRIPTION     Assist the Supports Coordinator in all areas of Person-Centered Plan including reviewing, documenting, and maintaining master files to ensure timely compliance in accordance with CARF, Department of Mental Health, Medicaid, or other funding source standards.  QUALIFICATIONS      Education: A minimum of an Associates Degree in Applied Science or related degree from an accredited college or university; and have a minimum of two years full-time responsible work experience in a human service agency; or be a high school graduate or have a certificate of completion of the General Educational Development (GED) test; AND have a minimum of four years full-time progressive work experience and responsibility in a human service agency.   Other:  1. Work and/or life experience with individuals with developmental, emotional and/or physical impairments valued.  2. Interpersonal skills sufficient to communicate with participants, public and co-workers.  3. Composition skills sufficient to prepare required documentation.  4. Must be at least 21 years of age.  5. Valid Michigan driver’s license; driving record must be sufficient to meet safe driving standards as established by Agency insurance carrier and will be run annually along with criminal background check, to comply with funding source requirements. DUTIES AND RESPONSIBILITIES   1. Under the supervision of a Supports coordinator, meet with persons served to monitor the implementation of the Person-Centered Plan and to review progress on goals and objectives, documented in monthly service notes.  2. Enter demographic, ability to pay and other data as required into the electronic medical record system.  3. Help to maintain master files and ensure timely compliance in accordance with CARF, Department of Mental Health, Medicaid, or other funding source standards.  4. Assist with clerical tasks such as filing, copying, compiling packets, as requested  5. Communicate with guardians, responsible relatives, caregivers and others involved in the life of the individual, as authorized.  6. Complete trainings as required by funding sources.  7. Identify and provide referrals as needed.  8. Driving participants in a personal or Agency vehicle as required.
