Qualifications: Licensed Practical Nurse or Registered Nurse and 2 years of experience in long term care management. Essential Job Functions: Direct, evaluate, and supervise all resident care and initiate corrective action as necessary. Assess resident care needs and assist in the development of individualized service plans of resident care. Evaluate resident care as related to individualized resident needs, family involvement, and the physicians orders for the resident. Participate in resident care conferences as needed. Coordinate with Assisted Living Manager for admissions and transfers.
