We are seeking an Associate Director of Community Care for a full-time position to join our Indiana Regional Office team in Indianapolis, Indiana. The Associate Director of Community Care works under the direction of the Regional Director of Transitional Healthcare and is a key member of the Transitional Healthcare Leadership Team. This role is responsible for providing oversight to the Transitional Healthcare Liaisons to see that there is continuation of care during formerly incarcerated patients' time on parole.
