About the Role: You will provide person-centred nursing care and case management while supporting the coordination of safe and effective patient discharge planning. Working closely with the Nurse Unit Manager and multidisciplinary teams, you will assist with service coordination, patient flow, practice improvement and day-to-day operational activities to support positive patient outcomes.
