Responsibilities

MAJOR DUTIES AND RESPONSIBILITIES OF THE POST:

Measurements for competences which you should be aiming to achieve, and which will be continuously reviewed are in shown below this list, which is not exhaustive.

  • Attendance at partnership meetings so that there is consistent representation at multi-agency meetings
  • Be approachable and supportive to provide first point of contact for ASRMs and health teams queries taking ownership to follow through to resolution where appropriate.
  • Co-ordinate and maintain information on the residents in the hotels, (if the resident’s consent to this).
  • Build positive relationships that promote signposting and or a two-way referral process with statutory services, other providers, charities and other groups e.g faith groups.
  • Support health services to have the information required for their visit / service by liaising with residents prior to the visit.
  • Maintain a source of health materials for the population including translated or pictorial information if required.
  • Educate patients on the pathways available in the UK for medical needs, such as Minor Injuries, Opticians, Dental, mental health resources including Talking Space Plus, Social Prescribing, Midwifery services, Health Visitor services self-referral for physiotherapy (if eligible), transport services, A &E, obtaining prescriptions (including exemptions)
  • Support residents to make appointments for services providing Hotel Clinics, explain appointment processes, and ensure they receive any appropriate information prior to their appointment. Clinics vary across the hotels they may include GP, Nurse, Phlebotomy.
  • Support with the booking of interpreters for health teams visits.
  • Have a physical presence in all contingency hotels across Oxfordshire.
  • Ensure residents are supported in any complaints process and highlight any themes to the ICB.
  • Ensure your communications are professional
  • Support residents to access follow up reviews i.e ensure transport can be accessed.
  • Assist with the gathering of statistics and information when required, being able to conduct searches.
  • Monitor ASRMs who leave and new arrivals to maintain up to date database. (With residents consent)
