Are you an experienced Care Coordinator, Senior Carer, Field Care Supervisor, Care Team Leader or Senior Caregiver who knows you’re capable of more? We’re working with a highly regarded and growing home care provider, based in attractive, modern premises in Gosport, who are looking for a dedicated and motivated Care Coordinator to join their team.

• Knowledge of PASS Care Management Software for Social Care would also be beneficial.

You don’t need years of previous Care Coordinator experience, now you’ll have the opportunity to use that experience in a different way, helping to coordinate high-quality care, support your colleagues and make a positive difference.

Your responsibilities will include:

Working alongside the Registered Care Manager, you’ll play an important role in ensuring clients receive safe, consistent and person-centred care, while supporting the care team who deliver it.

• Coordinating care staff and client visits.
• Assisting with rotas, scheduling and arranging cover.
• Supporting safeguarding procedures and maintaining high standards of care.
• Building positive relationships with clients, families and care professionals.
• Liaising with Social Workers, GP practices, NHS Discharge Teams and other key professionals.
• Supporting the Registered Care Manager with day-to-day coordination and office administration.
• Providing personal care in the community when required.
