Are you an experienced allied health professional looking for a new challenge? We are very excited to establish this new post for a Neuro-navigator in Enfield.
You will support patients with complex neuro rehabilitation needs as they move from acute services through to in-patient and community services, and also in supporting those with longer term neuro conditions . You will have an active caseload to support you to retain your clinical skills.
The role involves working in close partnership with acute services, specialist neuro rehab services, local rehab and community services in identifying the most appropriate patient pathway utilising inpatient rehab beds and community provision.
You will work alongside Adult Community Therapy colleagues and have peer support from other NCL neuro navigators in the North Central London Sector. You will be an integral member of the Adult Community Neurological Therapy Team based at St Michaels primary care centre EN1.
As a new post and a developing service, there will be ample opportunity for service development, and clinical leadership.
There is an established in-service training programme. Leadership and formal training opportunities are also available.
This post is for 32 hours per week Mon- Friday service and we are happy to discuss flexible working patterns and would welcome you to attend for an informal visit. With ongoing developments in community services there is the possibility of weekend working in the future.
North Mid is part of North Central London integrated care system – consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey. As with other ICS’s, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.
