BACHELOR'S LEVEL SOCIAL WORKER (BSW) ESSENTIAL FUNCTIONS * Assist patient in articulating personal goals for recovery and wellness through one-to-one and group sessions. * Provide support in identifying and creating goals and developing recovery plans with the skills, strengths, support, and resources needed to achieve goals. * Encourage and assist patient to collaborate with their Behavioral Health Professional and treatment team to plan steps for achieving their recovery and wellness goals. * Utilize tools to assist patient in creating their own individual wellness and recovery plans. * Work with the treatment team to teach problem-solving techniques to individuals and groups, encouraging patients to share and discuss strategies for coping and managing daily challenges. * Assist in building social skills that will enhance activities of daily living. * Collaborate with the re-entry team to help patient participate in the release planning process. * Assist in developing empowerment skills and combating stigma through self-advocacy. * Attend training and meetings as required. * Other duties as assigned. BACHELOR'S LEVEL SOCIAL WORKER (BSW) MINIMUM REQUIREMENTS * The Behavioral Health Professional co-leads group therapy, one on one therapy, and crisis therapy. * Must have a Bachelor's in Social Work * Preferred two-year employment in a social work capacity in a public or private agency or equivalent. * Must receive a satisfactory background investigation report.
