A key part of the role will be to ensure there is early communication with families/carers/care homes to ensure all relevant information is collated and potential barriers are highlighted so they can be addressed and solved prior to when the patient leaves hospital and mitigate failed discharge. Alongside these elements the post holder will be responsible for ensuring accurate data is captured on all data bases in order to fulfil all Trust reporting requirements
