Job Description As such, Direct Medics are working in partnership with our client based in Luton to find an experienced and dedicated Band 5 Pharmacy Technician to work within the Medicines Management Team. This role will be hybrid working however will require to attend domiciliary visits. Working hours will require 2-3 days per week (15-22.5 hours) on a temporary basis for 12 weeks. Duties: Domiciliary Visits: Conduct home visits for comprehensive medication management, including medicines reconciliation and immediate support. Investigation of Missing Controlled Drugs: Ensure safety and compliance by investigating incidents and implementing preventive measures. Huddle Participation: Triage discharge letters to resolve medication safety issues, improve medication optimisation, and reduce hospital readmissions. Medication Management: Collaborate with the wider CCS teams and adult social care services for effective medicines management. N.B. The post does not include dispensing of medication.
