  • Triaging new referrals from a number of sources including referral directly from patients, carers or professionals.
  • Provide clinical assessment, support, planning and oversight for patients who are in the community at risk of admission and/or placement breakdown.
  • Offer training and advice to carer’s, providers and health and social care staff specifically in relation to meeting the individuals’ needs and through their Positive Behavioural Support Plan.
  • Provide specialist advice and support to people to enable them to remain in the community and prevent, if possible, in-patient admissions.
  • Monitor/ support utilising our pathways including the Mental Health Pathway and Positive Behavioural Support Pathway until there is sustainable evidence that the risks are manageable/ reduced and if required handing over to LD Community teams for longer term monitoring
