Job Introduction
Wilton Community Team are seeking a Band 6 Specialist Community Nurse. Join our dynamic and supportive Wilton Community Team and help shape the future of community healthcare.
AS A SPECIALIST COMMUNITY NURSE, YOU WILL:
Provide nursing care, treatment, support, and advice to individuals in their own homes or outpatient settings.
Promote patient independence and wellbeing, working within national and local frameworks, professional guidelines, and organisational policies.
Deliver safe, person-centred care to a diverse caseload of patients with both short-term and long-term conditions.
Take clinical leadership responsibility for peers, junior trained staff, and support workers within your discipline.
Offer expert advice and guidance to other health and social care professionals.
Act as a key member of the team, contributing to service development and maintaining high standards of care.
What We’re Looking For:
A registered nurse (Adult) with current NMC registration.
Significant post-registration experience in a community or primary care setting.
Strong leadership skills and the ability to supervise and support junior staff.
Confidence in managing a caseload, prioritising work, and making complex clinical decisions.
A full UK driving licence and access to a vehicle (essential for community visits).

YOU WILL HAVE A FULL UK DRIVING LICENSE WITH ACCESS TO A CAR FOR WORK PURPOSES.
