CORE JOB REQUIREMENTS:

• To effectively liaise with a range of service providers both voluntary and statutory in relation to the assessment of referrals to the unit.
• To manage and triage referrals into Clinical Services and communicate effectively and professionally with Referral agents.
• To in some instances attend local hospitals (St. James University Hospital, Mater University Hospital etc.) to conduct pre- admission assessments.
• To meet and collect clients for admission from clinics or pharmacy.
• To liaise with the visiting G.P. and Nurse managers where necessary in relation to admissions and in addressing the health needs of all clients in the service.To work with the MDT in Clinical Services to coordinate and arrange effective, timely and appropriate discharges of clients from the service
