Summary: Implement and administer policies, programs and activities under the direction of a licensed behavioral health (BH) clinician. Provide support to 3-7 licensed BH clinicians. Responsible for providing social service referrals for patients and families, including housing, transportation, medical services, etc. Providing social service referrals and support. Ensure the preparation and maintenance of required documentation per accreditation standards and CMC's policy. Responsibilities: Work directly with patients and families as directed by licensed a BH clinician. Work effectively with PCPs and office staff to assure satisfaction with Children's BH services. Facilitate parent/child referrals to address social service needs and supportive therapy. Coordinate ongoing care with licensed providers and support groups. Support licensed behavioral health clinicians by outreaching families under their supervision. Assists in medication refills by transcribing requests into Epic and forwarding to prescriber for review and submission. How You’ll Be Successful: WORK EXPERIENCE At least 2 years previous experience in social services or healthcare required At least 4 years related experience preferred EDUCATION Four-year Bachelor's degree or equivalent experience required JOB PROFILE Requires experience in own professional discipline to appropriately apply own knowledge; broadens own professional expertise and acquires higher level skills Understands key business drivers; uses this understanding to accomplish own work No supervisory responsibilities but may provide informal guidance to new team members Solves problems in straightforward situations; analyzes possible solutions using professional/technical experience, judgment and precedents Impacts quality of own work and the work of others on the team; works within guidelines and policies Receives a moderate level of guidance and direction Explains complex information to others in straightforward situations A Place Where You Belong We put our people first.
