Location: North Hollywood & North Hills
Pay: $25.00 per hour
Schedule: Monday to Friday, day shift. Weekends as needed.
Healthcare professional needed to work as a Behavioral Health Care Coordinator. This position is responsible for being an integral part of a comprehensive behavioral health service team.
This role involves playing a pivotal role within an integrated system of care between primary care and mental health services, ensuring that patients are screened for mental health conditions such as anxiety, depression, and substance use.
The Behavioral Health Care Coordinator will work closely with clinic care teams to enhance patient-centered care and communicate patient’s needs and care plans to team members.
In addition, the Behavioral Health Care Coordinator will assist in facilitating transitions of care, addressing care gaps, conducting high-risk patient follow-ups, coordinating referrals, and screening for Social Determinants of Health (SDOH). This role also involves providing patient education for their care team panel.

QUALIFICATIONS:

  • Bachelor’s degree in nursing, social work, healthcare administration, or a related field.
  • Previous experience in care coordination, case management, or a related mental healthcare role.
  • Knowledge of healthcare systems, transitions of care, and patient-centered care principles.
  • Strong communication and interpersonal skills.
  • Bi-lingual English/Spanish preferred.

Responsibilities
  • Coordination of Care: Use external sources to outreach to patients with recent hospital or ED discharges related to a mental health diagnosis. Facilitate smooth transitions between healthcare settings, ensuring continuity and coordination of care. Coordinate appointments with mental health care providers to ensure timely delivery of risk assessment, intake, diagnosis, treatment, and follow-up.
  • Care Gap Outreach: Utilize outreach lists to address mental health and physical health gaps.
  • High-Risk Follow-ups: Perform brief consultations with patients with a high PHQ9 score. Work closely with medical providers and the care team to intervene with patients needing immediate consults due to high-acuity needs.
  • Referral Coordination: Manage the follow-up of external referrals for assigned providers’ panels, ensuring appropriate documentation and follow-up with the patient. Manage the department’s referrals by systematically enrolling and scheduling patients for mental health services.
  • SDOH Screening and Follow-up: Conduct screenings to identify social determinants impacting patients’ health and well-being. Work with mental health and medical providers to develop appropriate action plans.
  • Patient Education: Provide education to patients and their families regarding the importance of mental healthcare screenings. Utilize educational materials to empower patients to actively participate in their mental healthcare journey.
