Description

The purpose of this position is to work within an integrated system of care between primary care and mental health, ensuring that patients seeking medical services are screened for mental health conditions such as anxiety, depression and substance use, and provide assistance in navigating the systems of care (internal and with community partners). Utilizing Valley Community Healthcare’s (VCH) Patient Centered Medical Home protocols, the BHCM will engage with patients over the phone and in-person while working with clinic care teams to enhance patient centered care. The BHCM works as an integrated member of the interdisciplinary clinic team to communicate patient’s needs and care plans to the team members. The BHCM also helps patients navigate across the continuum of health care, including services received outside of VCH.

MINIMUM QUALIFICATIONS:

  1. Master’s or Ph.D/ Psy.D degree in social work, psychology or related field (MSW, MA, LMFT, Ph.D, or Psy.D)
  2. California license in social work or marriage and family therapy current/active (BBS)
  3. Three to five years minimum clinical experience required and supervisory experience preferred
  4. Knowledge of the Hispanic/Latino culture is a must
  5. Bilingual/Spanish Preferred
  6. Knowledge of integrated behavioral health; mental health recovery, harm reduction, age specific growth and development, crisis and behavior management is expected
  7. Working knowledge of administrative practices and procedures, HIPPA regulations, policies and standards related behavioral health

Responsibilities

PRIMARY RESPONSIBILITIES:

  1. Operations/Compliance
  2. Screening and diagnostic assessment using standardized tools and the DSM-5
  3. Provide evidenced based practices to individuals, couples, families, and groups
  4. Document in Electronic Health Records in a timely, accurate, and clinically sound way
  5. Submit billing daily that is accurate and complete
  6. Effectively communicate with all staff, volunteers, and community partners
  7. Provide patient advocacy as needed
  8. Meet productivity standards.
  9. Submit weekly caseloads.
  10. Maintain a caseload of up to 45 cases, 9 slots per workdays.
  11. Serve as MSW/ Ph.D/ Psy.D Field Instructor to social work graduate students, as needed and ASWs/IMF/ Ph.D, Psy.Ds as approved.
  12. Other duties as assigned:
    o Ensure that new patients and as appropriate, returning patients complete required screening tools;
    o Perform brief consultation with patients with high scores during their medical visit;
    o Work closely with medical providers and care team to intervene with patients needing immediate consults;
    o Perform triage services, crisis intervention, case management, referrals and follow-up as required;
    o Develop a collaborative relationship with primary care providers;
    o Participate in care team huddles and weekly clinical team meetings.
    o Participate in clinical supervision meetings;
    o Complete all required documentation by the end of each day
