Shawnee Health is seeking a Behavioral Health Care Manager for our Williamson County clinics.  This position reports to the Behavioral Health Program Coordinator. The Behavioral Health Care Manager functions as a core member of a collaborative care team that involves the patient’s primary care provider, psychiatric provider, behavioral health providers and other clinical team members in the primary care clinic. The care manager is responsible for coordinating and supporting behavioral health and substance abuse care provided in the primary care clinic. He/she is also responsible for coordinating referrals to clinically indicated services outside the primary care clinic (e.g., social services, mental health specialty care, and substance abuse treatment). Evidence-based screenings and other activities in support of the primary care provider directed care.     Responsibilities * Support and closely coordinate behavioral health and substance abuse care with the patient’s primary care provider and, when appropriate, other treating behavioral health providers. * Screens patients for common behavioral health and substance abuse disorders utilizing evidence based tools. * Provide patient education about common behavioral/mental health and substance abuse disorders and available treatment options. * Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complication as reported by patient. * Support psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment. * Provide brief interventions using evidence‐based techniques such as problem‐solving, motivational interviewing, or other interventions appropriate for primary care settings. * Facilitate in‐clinic or outside referrals to behavioral health and substance abuse treatment as clinically indicated. * Facilitate patient engagement and follow‐up in care. * Track patient follow up and clinical outcomes in EHR. Document in-person and telephone encounters and use the system to identify and re-engage patients. * Document patient progress in the EHR so that they can be easily shared with PCPs, the BHS, psychiatrist, and other treating providers. * Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP. * Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment). * Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and have been discharged from the caseload. Requirements * Master’s degree in related field and additional 2 years behavioral experience is preferred.
