Description Position Summary: Provide client centered management assistance, services, and resources to Behavioral Health Department and Patients. Position responsibilities: Provides resource information to patient population for Behavioral Health. Establish rapport and professional relationships with area agencies and patients. Provide direct linkage services to patients with area resources. Collaborate with providers, healthcare professionals, social service agencies and support staff. Keep timely and accurate service documentation. Makes calls, emails, and faxes as needed for the Behavioral Health Department. Advocate on behalf of patients in need to outside agencies. Assess patients’ needs and support systems. Planning for crises and helping clients develop coping mechanisms. Complete Behavioral Health Department related tasks as deemed appropriate by the Behavioral Health Supervisor. Other duties as assigned. Requirements Essential skills and experience: Bachelor’s Degree in Social Work, Psychology, Sociology, Counseling, or related field. Two years of experience with a mental health agency and case management. Excellent interpersonal and communication skills. Problem analysis and problem resolution at both a strategic and functional level.
