Description Make a real impact in behavioral health, primary care, and substance use (MAT) at Konza Prairie Community Health Center. We’re hiring a compassionate LPN to support providers, coordinate care, and improve patient outcomes in a fast-paced, team-based clinic. Key Responsibilities Patient intake, rooming, vital signs, and assisting with exams/treatments Nursing triage (phone calls, symptom assessment, urgent vs. non-urgent) Administer medications, injections, and support treatment plans Coordinate referrals, prior authorizations, diagnostic testing, and follow-ups Support behavioral health and substance use disorder (MAT) patients Monitor medication adherence, compliance, and relapse prevention needs Manage prescriptions and refills with pharmacies Document accurately in EHR/EMR (eClinicalWorks) Provide patient education on medications and care plans Requirements Active, unencumbered Kansas LPN license BLS (Basic Life Support) certification Vaccinations per ACIP healthcare guidelines Preferred Qualifications Experience in behavioral health, mental health, substance use disorder, MAT, or primary care Experience with triage, care coordination, or case management Bilingual (English/Spanish) At Konza Prairie Community Health Center, we foster a culture of compassion, growth, and community impact.
