Description Konza Prairie Community Health Center is seeking a compassionate and motivated Behavioral Health Nurse (LPN) to join our integrated care team. This position plays a vital role in supporting patients receiving behavioral health and substance use disorder services by providing direct patient care, coordinating treatment, and partnering closely with providers to improve patient outcomes. As a Behavioral Health Nurse, you'll work in a collaborative environment where medical and behavioral health services come together to provide whole-person care. This position offers the opportunity to build meaningful relationships with patients while making a lasting impact in the communities we serve. What You'll Do Provide compassionate nursing care to patients receiving behavioral health and substance use disorder services. Prepare patients for appointments by obtaining vital signs, gathering health information, and assisting providers during exams and treatments. Conduct telephone triage and coordinate follow-up care based on patient needs. Administer medications and injections as directed by providers. Coordinate medication refills, prior authorizations, referrals, and communication with pharmacies. Monitor patient progress and support treatment compliance through ongoing education and care coordination. Maintain accurate documentation and ensure compliance with clinical, safety, and regulatory standards. Collaborate with providers and the care team to deliver high-quality, patient-centered care. Current BLS certification for Healthcare Providers. Current immunization record meeting ACIP Healthcare Worker recommendations. Excellent communication, organizational, and critical thinking skills. Ability to remain calm and effective in fast-paced or emergency situations. Preferred: 5 or more years of nursing experience in Family Practice, Behavioral Health, or Substance Use Disorder treatment. Experience with medication-assisted treatment (MAT) and behavioral health populations. Bilingual (English/Spanish).
