Description

Michael’s House is currently seeking a dynamic Clinical Care Coordinator - Full Time. The Clinical Care Coordinator plays a critical role in providing integrated case management and substance use disorder counseling services to clients in a Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) setting. This position supports clients in achieving recovery goals by delivering evidence-based interventions, coordinating care, and providing psychoeducation in a trauma-informed and person-centered environment.

EDUCATION AND EXPERIENCE

  • Active CADC-I, CADC-II, or CADC-III certification from CCAPP, or equivalent certification from CAADE or CADTP. - Required
  • Minimum of 1 year of experience in a substance use disorder treatment setting. - Required
  • Knowledge of ASAM Criteria and evidence-based treatment practices. - Required
  • Strong understanding of confidentiality regulations (42 CFR Part 2, HIPAA). - Required
  • Proficient in clinical documentation and electronic health records (EHR). - Required
  • Bachelor’s degree in Addiction Studies, Social Work, Psychology, or related field. - Preferred
  • Bilingual (English/Spanish or other languages) preferred.
  • Experience in a PHP/IOP setting. - Preferred
  • Knowledge of co-occurring disorders treatment. - Preferred

Responsibilities
  • Conduct comprehensive biopsychosocial assessments and develop individualized treatment plans aligned with ASAM Criteria.
  • Provide ongoing case management services including care coordination, referrals to community resources, and discharge planning.
  • Facilitate individual counseling sessions and psychoeducational groups on substance use recovery, relapse prevention, coping skills, and related topics.
  • Co-facilitate process groups focused on peer support, emotional regulation, and recovery maintenance.
