The Associate Clinical Social Worker (ACSW) will be a key member of a multidisciplinary Street Medicine Team providing direct outreach, clinical support, and substance use services to individuals experiencing sheltered and unsheltered homelessness. The ACSW will work alongside medical providers, community health workers, harm reduction staff, and the street medicine team in general to deliver trauma-informed, person-centered care directly in encampments, shelters, and community settings. Experience: (Required) * 2 years providing substance use disorder treatment counseling or as an ACSW and supports in an integrated health setting. * Bilingual (preferred). Licensure/Certification: * Registered with the California Board of Behavioral Sciences as ACSW, MFTi Responsibilities: Street-Based Clinical Services * Conduct field-based biopsychosocial assessments, substance use assessments (including harm reduction and MAT readiness), and crisis risk evaluations. * Provide brief counseling, Motivational Interviewing, relapse prevention, harm reduction education, and supportive casework in nontraditional outdoor settings. * Build trust with clients through consistent, low-barrier engagement in encampments, on the street, and in shelters. * Support clients in navigating detox, residential treatment, MAT, and outpatient behavioral health services. * Assist in crisis intervention, de-escalation, and safety planning in the field. Care Coordination & Outreach * Collaborate closely with providers, nurses, substance use navigators, community partners, housing organizations, and SJCH Programs such as DV, RISE PRIME, ECM/CS, and others. * Coordinate referrals for medical care, mental health treatment, substance use programs, basic needs resources, legal services, and SJCH Programs such as DV, RISE PRIME, ECM/CS, etc. * Engage in street outreach to identify and connect with high-need individuals who are disconnected from traditional services. * Advocate for client needs within health, behavioral health, and housing systems. Documentation & Compliance * Complete timely and accurate documentation in accordance with Medi-Cal, county, and SJCH electronic health records on ECW.         * Maintain field notes, assessments, care plans, and progress notes using mobile technology when in the field. * Participate in clinical supervision, team meetings, case conferences, and training sessions. Program Support * Assist in developing outreach strategies, group interventions, and harm reduction programming specific to street-based populations. * Work closely with other program with other SJCH programs and help provide services to patients.  * Support the Street Medicine team in general operational activities such as loading vans, creating patients’ bags, packing snacks, recruiting patients assisting with program referrals and others.  * Participate in data collection, quality improvement initiatives, and program evaluation efforts. * Contribute to a positive, collaborative team culture focused on compassion, dignity, and reducing barriers to care.
