We offer primary care, behavioral health, substance use disorder treatment, and community support services. Duties & Responsibilities Provide triage for behavioral health and support crisis and emergency situations. Deliver crisis intervention and individual counseling services; manage assigned caseload. Serve as a “bridge” for integrated care, including warm handoffs and patient check-ins with medical providers. Conduct mental health assessments and develop treatment plans. Support primary care providers, nurses, and other team members with consultation. Engage with cross-agency treatment and prevention teams in the community. Record patient case information in the EMR within 72 hours of contact. Maintain compliance with HIPAA, ethical standards, and NH state laws regarding crisis management and involuntary admittance. Requirements EDUCATION/EXPERIENCE: A master's degree in: Clinical Psychology, Clinical Mental Health Counseling, Social Work, or Marriage and Family Counseling.
