JOB SPECIFICS

  • Location: 301 E St Joseph St, Green Bay, WI 54301
  • Hours: 32 hours/week
  • FTE Status: 0.8 FTE, Full-Time
  • Work Schedule: Mix of day and p.m. shifts, with an every other weekend requirement; Day shift - 7:00am - 3:30pm, P.m. shift - 3:00pm - 11:30pm

JOB DESCRIPTION:

  • Responsible for case review and telephone communications with patients for assessment, and assistance with appropriate level of care needs and/or safety planning if appropriate.
  • Responsible for answering phone calls, telephone assessment and triage, and scheduling full patient assessments with appropriate level of care resource.
  • Functions as a member of the multi-disciplinary treatment team.
  • Responsible for communications across entire health system related to patient referrals and outcomes.

QUALIFICATIONS:

  • Bachelor’s degree in Social Work, Psychology, Counseling, or related field required.
  • Previous experience in triaging required.
  • Wisconsin Social Work or RN license preferred.
