Make a Difference Gallahue Mental Health Services/ Community Support Services has a full-time Ambulatory LPN position available. Community Support Services Clinic is located at 5470 E. 16th. Street in Indianapolis, IN. The Behavioral Health Ambulatory LPN works hand- in- hand with the Physician to achieve quality outcomes for the patient. The Ambulatory LPN will assist the provider of a health care site with facilitation of patient visits and procedures, routine laboratory tests, referrals, and quality outcomes for the population of the practice. The Behavioral Health Ambulatory LPN will administer long-acting injectable medications to those receiving services in the Serious Mental Illness (SMI) clinics. There is some travel to and from group homes (less than 4 miles away) for medication management. Exceptional Skills and Qualifications Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience. (Required) Previous experience in medical office or health care setting. (Preferred) Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA)
Responsibilities
The Behavioral Health Ambulatory LPN works closely with the Physician to facilitate patient visits and procedures, ensuring quality outcomes. They will also administer long-acting injectable medications to patients in Serious Mental Illness clinics and manage medication for group home residents.
