KNOWLEDGE, SKILLS, AND ABILITIES:

MINIMUM QUALIFICATIONS FOR CONSIDERATION:

  • Bachelor’s degree from an accredited university.
  • Certification as a Behavioral Health Case Manager.
  • At least two years experience in the behavioral health field.
  • Crisis service experience preferred

PHYSICAL REQUIREMENTS:

  • Sitting and Standing for prolonged periods of time.
  • Bending, stretching, kneeling and may require some lifting
  • Repetitive computer work
  • Must possess a sound physical and emotional personality
  • Emotional demands are particularly heavy because of the personal and sensitive nature of the work
  • Performance of duties often involves stamina and alertness
Responsibilities

ESSENTIAL FUNCTIONS:

  • Provides planned linkage, advocacy, and referral assistance to behavioral health clients.
  • Supports continuum of care efforts aimed to strengthen self-sufficiency and improve the quality-of-life experiences.
  • Serve as point of contact for clinic crisis referrals, initiates and facilitates crisis response processes and required documentation, prioritizes coordination of care
  • Schedule and monitor crisis follow up services
  • Works in accordance with treatment plan goals and objectives developed in partnership with client and clinical staff.
  • Participation, facilitation, and documentation of clinical staffing with the coordinator of Behavioral Health/Substance Abuse.
  • Maintains professional boundaries and adheres to credential competencies in work to advance the welfare of individuals, children, and their family.
  • Utilize best practices in documentation and record keeping .
  • Records weekly client call log
  • Works collaboratively with clinical staff in planning and facilitating educational groups.
  • Performs community outreach and maintains community connections needed to ease access of resources needed by clients.
  • Performs weekly follow up calls and works to fill gaps in services.
  • Supports administrative staff with tracking and scheduling appointments and providing office coverage as needed.

ADDITIONAL DUTIES:

  • Perform other duties and projects as required by management.
  • Maintain collaborative partnership with trial police in crisis response work
  • Manage flexible work schedule to provide crisis work as needed.
