POSITION SUMMARY:

The Behavioural Support Lead serves a dedicated role as an in-house Lead on identification and management of responsive behaviours associated with dementia, complex mental health, substance use, and related neurological conditions. The person in the role will lead the Home to work with the Behavioural Support Team (BST) hospitals, primary care providers, and other providers and professionals. The Lead will participate in Central LHIN’s Behavioural Support Services Program

QUALIFICATIONS:

  • Registered Practical Nurse with a current certificate of performance and in good standing with the College of Nurses of Ontario;
  • Possession of valid CPR, and First Aid Certificates;
  • Certification in Gerontology and current membership with GNA and RNAO is desirable;
  • Minimum of one year experience in geriatric, rehabilitation or long-term care preferred;
  • Adherence to professional competencies, i.e. caring and respectful behavior, clinical knowledge and skills, assessment skills, communication skills, conflict management skills and organization skills;
  • Demonstrated supervisory and team leadership skills;
  • Proficiency in oral and written English; ability to speak Cantonese/Mandarin is an asset;
  • Willingness to work rotating Day/Evening/Night shifts with alternate weekends;
  • Relevant computer literacy;

Responsibilities
  • Provide leadership and support to Home in the management of responsive behaviours.
  • Maintain an active clinical caseload of residents with responsive behaviours.
  • Convene and lead regular Behaviour Rounds to enable front line staff to discuss care plans with internal and external resources.
  • Provide proactive assistance to residents and staff during transitions between care settings to reduce the risks of responsive behaviours.
  • Develop and maintain special knowledge and skills on the assessment and management of responsive behaviours related to these conditions.
  • Develop and maintain knowledge and skills related to the use of frameworks such as GPA, PIECES and UFirst.
  • Develop and maintain knowledge and skills in the application of these frameworks for the assessment and care planning for residents at risk of/or presenting with responsive behaviours.
  • Collaborate with the Psychogeriatric Resource Consultant (PRC) in identifying knowledge-to-practice needs and building the Home’s capacity to deliver comprehensive, quality resident and family centered responsive behaviour services.
  • Optimize the engagement of external resources such as Mackenzie Health Mobile Team, Ontario Shores Mental Health Outreach team, and Home and Community Care Providers and/or other applicable resources.
  • Assist in the translation of the recommendations of external resources into care plans and coach the implementation of these care plans.
  • Ensure effective monitoring of the effects of the care plans with the use of such tools as the Dementia Observation Scale, the Team Debrief Tool and clearly document assessment findings, care plans, intervention results etc. in resident records.
  • Provide coaching, training and support to direct care staff in the management of resident’s responsive behaviours.
  • Maintain accurate activity, service indicator data, statistics and reports as requested for routine organizational reporting to the LHIN’s Behavioural Support Services Program
  • Maintain effective relationships with multidisciplinary team members and local leaders that enable the identification and resolution of local and systemic issues.
  • Participate in best practice programs and continuing to enhance professional development to support the behavioural challenges of residents.
