Job Description Hanrahan Health are excited to announce that we are expanding the range of supports we provide to our clients by adding positive behaviour support! We are looking for an experienced Behaviour Support Practitioner with current suitability at the Proficient or Advanced level who is driven to provide an exceptional standard of evidence-based, person-centred and outcome-focused positive behaviour support to NDIS participants. On a daily basis you will be: Assisting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning Evaluating, developing and delivering evidence-based support plans within a complex support space Developing meaningful goals and practical strategies utilising a positive behaviour support framework, and with a commitment to eliminate restrictive practices, in the home and community Ensuring individual plans reflect our NDIS participants' goals, passions and strengths Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc Working collaboratively within a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners.
