To help fulfill its mission, LOFT is hiring a Full Time Contract Behavioural Personal Support Worker.

JOB SUMMARY

The Behavioural Personal Support Worker (BPSW) is part of a dedicated team to the @Home Programs which will provide direct observation, care, education and support to clients demonstrating responsive behaviours and their caregivers who are transitioning back to their own home from the hospital. This position will also assist in the assessment and development of a Behavioural Support Plan. Working within a team of other professionals, the BPSW will enhance the quality of life for clients with responsive behaviours related to mental health, addictions, dementia or other forms of neurological conditions. In addition, the BPSW will assist and provide on-going training for clients, families and other health care providers.
The @Home Behaviour Support team provides direct care, education and support to older adults and their care partners, who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia, mental health, substance use or other neurological conditions who are being discharged from hospital within North Toronto, York Region and South Simcoe. The team completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Working as a member of an integrated team along side other home and community-care providers, the @Home team supports the transition from hospital and works within the individuals own home.

Responsibilities

This position supports the physical health, responsive behaviour, and dignity of the clients, thereby improving their quality of life by using the agreed upon strategies to address responsive behaviours by:

  • Act as the Primary PSW for assigned client(s). Follow the weekly schedule of required visits
  • Perform and record complete and accurate observation of client and their responsive behaviour during initials visits after being enrolled in the program. Assist in identifying triggers for the responsive behaviour
  • Documents activity according to agency standards in client information system and reports observational data using specified tools (ABO’s, Standard Behavior Questionnaire etc.)
  • Assists the Psychogeriatric Case Manager and Behaviour Support Specialist in the development of a Behavioural Support Plan towards the management of responsive behaviours
  • Offers suggestions to the development of Behavioural Support Plan with special emphasis on strategies to manage responsive behaviours
  • Actively practice the suggested interventions and continually adapt the interventions and strategies to ensure the best possible outcome for mitigating the responsive behaviours
  • Uses the agreed upon strategies to address responsive behaviors and reports to other team members when responsive behaviors are escalating
  • Model the effective interventions/strategies to caregivers and/or family members using peer-to-peer mentoring
  • Attend Behavioural Support Plan meetings/Discharge meetings with clients and their caregivers
  • Participates in multi-disciplinary conferences, client care conferences, and staff development conferences
  • Shares responsibilities for client care by working together with other staff members
  • Meets the needs of clients by communication, observation, and direct care
  • Reports any changes in conditions or abilities as well as any observations to the team as per the Health Service Provider protocols
  • Works in compliance with the Health and Safety Act and its regulations in performing duties in a safe manner and follows all health and safety policies, procedures, and legislation
  • Follows standard infection control practices and enhanced precautions as instructed
  • Complete other duties, from time to time, as assigned by the Director of Behavioural Support Services and/or Program Director
