Description Job Summary: Maple City Health Care Center (MCHCC) is a Federally Qualified Health Center dedicated to improving our community’s health by making quality comprehensive healthcare accessible to all. The Behavioral Social Worker is a member of the behavioral health team that supports the work of a cross-disciplinary care team that includes health care providers, nurses, behavioral health providers, and other support staff. The Behavioral Health Advocate supports the mission and values of Maple City Health Care Center (MCHCC) by working with patients and their care teams to improve behavioral health aspects of their care. Duties and Responsibilities: Collaborates with patients, Behavioral Health team, and medical team on various aspects of patient care.
