Responsibilities:
To secure, manage, and maintain all benefits and entitlements for individuals supported in the DD Residential Programs, ensuring timely access to critical resources that promote stability, well-being, and independence.

Core Responsibilities:

  • Ensure 100% of eligible individuals have accurate and complete applications submitted for Social Security, Medicaid, SNAP, and VA benefits within 30 days of program admission or eligibility determination.
  • Achieve a 95% on-time completion rate for renewals and recertifications by tracking and responding to HRA agency requests to avoid any lapse in entitlements.
  • Successfully request and represent cases in Fair Hearings for denied or lapsed benefits, aiming to reinstate 90% of benefits through effective appeals and timely submissions.
  • Ensure 100% of application submissions are supported with required documentation (e.g., PHI, work history, appointment verification) by maintaining ongoing communication and coordination with residential program staff.
  • Report all address or county changes to HRA agencies within 5 business days of notification to prevent benefit interruptions and ensure records remain up to date.
  • Request replacement benefit cards (Medicaid, SNAP, etc.) within 3 business days of report to ensure individuals supported have uninterrupted access to entitlements.
  • Achieve 100% documentation of all benefit applications, updates, correspondence, and renewals in the Electronic Health Record (EHR) within 48 hours of any action to support compliance and audit readiness.

Qualifications:

  • High School or GED
  • 3 years+ experience working with HRA agencies
  • Highly organized and detail oriented with great record keeping skills
  • Computer literate with Word, Excel and Adobe and working knowledge of Evolv/EHR system
