POSITION SUMMARY Working within an inter-professional team, the Bereavement Coordinator (Children & Youth Programs) assesses client suitability for various service programs that support client wellness in the Niagara community.   The Bereavement Coordinator (Children & Youth Programs is responsible for the intake and assessment for all children and youth for Hospice Niagara’s programs and services, as well as supporting the Bereavement Specialist and Volunteers with the delivery of a variety of programs, evaluating program effectiveness, coordinating key events and maintaining a client information database.    The Bereavement Coordinator (Children & Youth Programs performs all duties in a safe and healthy manner, in accordance with Hospice Niagara’s policies and procedures, the Occupational Health and Safety Act, and all other applicable legislation.     KEY RESPONSIBILITIES Client Support * Ensuring child and youth clients are appropriately triaged, assessed and have completed the intake process prior to participation in the identified program or service * Coordinating support programs, with an emphasis on identifying and addressing resource needs through the utilization of skilled volunteers * Coordinate and maintain weekly programs in various locations and formats (in-person or virtual) * Coordinate and oversee adjunct program activities and therapies (e.g. art, music, etc.)   Program Development & Reporting * Overseeing the content of community child and youth program materials and coordinating events in collaboration with other teams and service providers * Developing program outcome reports; overseeing annual surveys and contributing to continuous improvement initiatives * Provide input to achieve accreditation standards and ensure program standards are continuously maintained * Producing program evaluations, statistical reports, budgets and grant proposals * Oversee program accreditation * Maintaining an accurate client database General Responsibilities * Mentoring and overseeing staff, volunteers and students who are responsible for facilitating community programs * Problem-solving and addressing program issues related to volunteer support * Regularly check inventory and maintaining appropriate supplies and program space * Participate in and/or leads volunteer training during relevant sessions * Participating on internal committees and projects * Provide backfill to other program staff during vacation, illness, etc. * Perform other job-related duties, as assigned * Ensuring goals and objectives are developed and executed in alignment with the vision, mission, values and strategic priorities of Hospice Niagara SKILLS & ATTRIBUTES * Experienced in group grief counselling with a specialized focus on children and youth * Experience in developing and facilitating engaging and supportive virtual bereavement groups * Demonstrated understanding of clinical intake processes to triage and refer for appropriate services * Demonstrated knowledge of grant writing and reporting requirements including data analysis * Exhibit a professional, compassionate and caring approach * Demonstrate strong active listening and interpersonal communication skills * Ability to develop relationships based on trust, compassion and empathy * Effectively set appropriate boundaries and understand the limitations of volunteers in client care * Demonstrate a high degree of self-direction and self-motivation * Demonstrate a healthy adjustment to loss and exercise ongoing self-care   ACADEMIC, TRAINING, KNOWLEDGE & EXPERIENCE  * Clinical based professional (nursing or social work or related field) with supervisory experience required. * Vulnerable Sector Police Check (favourable, within past 12 months) * Fundamentals of Palliative Care (preferred, must be completed within 12 months of hire) * First Aid/CPR (preferred, must be taken within 12 months of hire) * Mask Fit (preferred, must be completed within 12 months of hire) * Safe Food Handling (preferred, must be completed within 12 months of hire)    Evaluation  * Ongoing observation and feedback * Annual performance assessment and discussion; individual development planning
