Title: Regional HSPRS Case Manager Location: Florida, USA Schedule: Hybrid, Monday to Friday (8:30am-5:00pm) requires traveling and covering different states. Summary:  Under the general guidance of the Regional Manager, the Regional Home Study and Post-Release (HSPRS) Case Manager provides a variety of specialized services related to the processes of conducting home studies and providing post-release case management. BCBS PPO, HMO, and HDHP with HSA Plan   * Dental insurance.     * Vision insurance.     * 401(k) 3% Employer Contribution.       * Commuter benefits.   * Professional Development Assistance.   * Disability insurance. * Voluntary Benefits.     * Flexible Spending Account.   * Life/AD&D Insurance. * Employee Assistance Program. * Special Vendor Discounted Offerings on Travel, Amusement Parks, etc. Essential Functions: Duties/Responsibilities: * Assesses and provides a recommendation for the purpose of reunifying children with their potential sponsors, including conducting interviews and background checks and documenting findings in a comprehensive report within timelines established by funding body. * Completes home visits to meet with children and their sponsor's post-release. * Coordinates referrals and service planning in relevant areas. * Works with children and their sponsors to encourage and ensure progress through their individual service plans. * Continually assesses ongoing changes in behavior and general conditions that could impact the safety of a participant. * Builds and maintains relationships with community services providers in relevant areas. * Serves as point person for stakeholders in each family’s case, including legal providers. * Participates in regular case management and staffing meetings. * Maintains accurate records and consistently ensures the confidentiality of information relevant to children and their cases. * Maintains records of post release services including measurable outcomes and justification for termination of services. * Submits required reports and documentation in a timely manner. * Provides crisis intervention when necessary, including involving Child Protective Services. * Travels both locally and out of state for the purpose of conducting home visits depending on their assigned caseload. * Other duties as assigned. Education and Experience: * Bachelor of Social Work (BSW) degree or an equivalent degree in education, psychology, sociology, or other behavioral science equivalent required.   * Minimum one (1) year experience in family reunification, home assessment and report writing, family preservation and/or general child welfare required.   Certifications/Licenses: * Valid and in good standing driver’s license (or the ability to obtain a driver’s license within 15 days of employment). Knowledge/Skills/Abilities (K/S/A) * Ability to speak and fluency in a second language including but not limited to the following multi-language abilities: * Ability to read and interpret documents such operating and maintenance instructions, and procedure manuals.   * Ability to write routine reports and correspondence.   * Ability to speak effectively before groups of customers or employees of organization. * Access to regular, dependable and timely transportation  * Ability and comfortability working with people of varying socioeconomic backgrounds. * Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders. * Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner. * A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability. * Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance. * Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. * Ability to work independently and remain self-motivated with minimal direct supervision. * Strategic thought partnership and consultancy skills. * Multi-language knowledge and/or ability. Work Arrangement:   * This position will be a hybrid role for HHCS and will have daily work hours/work schedule of. M-F, 8:30am – 5:00pm, to complete work and all related necessary tasks. This position may need to physically attend meetings, site visits, and complete other administrative duties on site. It is expected this position be able to accommodate the in-person site needs during the scheduled work week. * It is expected this person would be available for in-home visits.  Physical Demands:  * While performing the duties of this job, the employee is regularly required to talk and hear.   * The employee is regularly required to sit, stand and walk. * The employee must be able to stoop, kneel and/or crawl. * The employee is regularly required to use hands to key, handle, or feel and reach with hands and arms.  * The employee must occasionally lift and/or move up to 25 pounds.  * The employee must have unrestricted ability to provide physical restraint. * Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
