We are seeking a Community Health Worker (CHW) with Medical Assistant (MA) experience to join our team. This role is a unique blend of patient outreach, education, and clinical support. The ideal candidate will have strong communication skills, clinical knowledge, and a passion for helping patients navigate their healthcare journey.

QUALIFICATIONS:

• Medical Assistant certification and clinical experience required.
• Community Health Worker certification or relevant experience preferred.
• Strong interpersonal and communication skills.
• Bilingual (English/Spanish) preferred but not required.
• Ability to work independently and as part of a team.
• Knowledge of community resources and healthcare navigation a plus.

• Provide health education, resources, and support to patients and families.
• Assist patients in accessing medical care, social services, and community resources.
• Conduct patient follow-up calls and document outcomes.
• Perform basic clinical duties such as taking vitals, administering injections, and assisting providers with procedures (as permitted by certification).
• Support patient intake, charting, and documentation in the EMR system.
• Participate in community health events and outreach programs.
• Collaborate with providers, care coordinators, and administrative staff to improve patient care.
