Key Responsibilities Implement engaging health education programming in both individual and group settings to enhance patient knowledge and wellness. Utilize techniques such as motivational interviewing and disease self-management to support patient health goals. Provide information, referrals, and assistance for linkage to services, promoting patient independence and health. Conduct point-of-care testing for HIV and other conditions in community and clinic settings. Contribute to the development of innovative health education programs that make a difference. Collaborate as a member of an interdisciplinary clinic and care team, where teamwork is key. Perform brief assessments of patient needs and readiness for health behavior change. Establish and maintain referral processes and community networks. Document all patient services in electronic health records and data systems with precision and care. Minimum Qualifications High School Diploma or equivalent and 3 - 5 years of relevant experience. Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred. Performing point-of-care testing to clients in a clinical or community setting. Demonstrated actions and activities with respect to HIV care and prevention. Provides examples of performing education to clients of diverse cultures and backgrounds. Phlebotomy trained preferred. Bilingual English/Spanish highly preferred. Knowledge of word/excel and other computer programs.
