ONSITE POSITION

South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support, and advocacy to address the social determinants of health. SEOCHC particularly responds to people who face barriers to accessing health and social services and who live in Southeast Ottawa. Our Primary Care clinic is seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our interprofessional primary care team. The RPN will work closely with physicians, nurse practitioners, registered nurses, social workers, and other allied health professionals to deliver high-quality, evidence-based care to clients across the lifespan.

QUALIFICATIONS

  • Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).
  • Minimum 2-3 years of experience in primary care, community health, or related setting preferred.
  • Experience working with marginalized populations, including individuals with complex health and social needs.
  • Proficiency with electronic medical records (EMRs), preferably PS Suite.
  • Fluency in English and French language preferred; other languages representing our community (Arabic, Somali) is a strong asset.
  • Ability to work flexible hours including evening and weekends.
    Salary range: $47,902.40 to $56,347.20/annum

Responsibilities
  • Provide comprehensive nursing care within the RPN scope of practice, including health assessments, immunizations, wound care, chronic disease management, and medication administration.
  • Support clinical procedures and assist in minor medical interventions.
  • Collaborate with the healthcare team to develop, implement, and evaluate individualized care plans.
  • Promote health education and self-management strategies for clients and families.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Participate in quality improvement initiatives and team meetings.
  • Reach out to clients to assess urgent situations and determine the most appropriate course of action in collaboration with the on-duty physician.
