Description Reports To: Behavioral Health Director Employee Status: Exempt (Regarding Overtime) Position Summary: Under general supervision, but in line with established PC&HC policies and procedures, diagnose mental illness, prescribe medication, and provide counseling and intervention services to patients in order to improve psychosocial functioning. Essential Functions Represent PC&HC to patients/visitors in a caring, courteous, and professional manner. Provide prompt, efficient and accurate patient care. Review patient cases with collaborating Physician and utilizes recommendations as needed. Provide behavioral health services to the health center patients under protocols developed by the Medical Director/collaborating Physician and CEO. Collaborate with the CEO and Medical Director in the development/update of practice protocols, policies and procedures. Address client mental healthcare needs by performing assessments, treatments and medication interventions. Work closely with the other providers of PC&HC to deliver an integrated model of patient care to the patient population. Provide brief intervention, assessment and consultation at request of Providers. Counsel and educate patients on personal issues, such as mental illness, abuse and addiction, poverty and disease management to improve the client’s overall well-being. Evaluate crisis situations; intervene and treat appropriately. Interview families, individuals and groups and assess their needs. Conduct individual, family and group therapy, outreach and crisis intervention. Perform psychosocial evaluations and develop a plan of care to help client be successful. Assist in detecting clients who are “at risk” and develop a plan to prevent further deterioration. Continuously evaluate quality of care, outcomes of treatment, and utilization of services to prevent misuse of resources. Facilitate patient’s progress through the health care system including coordination, integration and direct delivery of patient services. Develop and maintain a community referral base for the purpose of referring patients to additional services in a timely and appropriate manner. Conduct surveys and studies relating to psychosocial status and patterns of population groups as directed. Interpret laboratory and diagnostic findings, taking action as needed. Complete documentation on all patients using E-Clinical software within 72 hours of patient encounter. Review patient health records. Develop and utilize referral relationships with physicians, hospitals, hospitalist, specialist, etc. Address patients RX request and other needs per the Electronic Medical Records within 24-48 hours of request. Assist with the training of other behavioral staff as assigned per clinic management staff. Ensure the infection control standards and OSHA standards are followed and hazardous waste is properly disposed. Plan and organize work load to maintain efficient patient operations in the Health Center. Maintain a strong patient focus while working collaboratively within the teams multiple demands and patient needs. Practice Patient Centered Medical Home Concept. Treat all patient information confidentially and in accordance with established guidelines. Improve the patient experience and health care processes and help assure the provision of high quality health care, as well as an ongoing process of performance improvement. Marginal Functions Perform all other duties as required or assigned. Requirements Requirements Must hold a valid NP Tennessee license. Must have a Board Certification. Must be able to lift 30 pounds/ walk and/or stand for up to 10 hour shifts-bend or stoop throughout shift Must attend conferences, seminars, and/or classes yearly which focus on current practices of Mental Health Nurse Practitioner. Must maintain all licensure and certifications to perform duties as required. Must follow all HIPAA and OSHA requirements.
