PURPOSE: We are currently hiring a Nurse Practitioner to join our CloseKnit team to deliver hybrid in-person and virtual care. CloseKnit is a virtual first advanced primary care practice looking for an experienced provider interested in innovating in the practice of primary care to deliver on the quadruple aim in a value-based arrangement for adult and pediatric patients. While providing both in person and virtual primary care services, the nurse practitioner will be working with the CloseKnit team to optimize the care delivery strategy and best practices in delivering a hybrid model of care to our patients. DUTIES: Primary Care Services Maintain a panel of adult and pediatric patients who receive high quality, attentive, longitudinal in person and virtual primary care. Provide comprehensive primary care including diagnosis, treatment, documentation, and minor procedures for acute, preventive and chronic patient needs. Use evidence-based medicine where possible and provide triple aim-high quality, low cost and excellent patient experience care delivery. Perform physical exams and medical procedures for patients who have a virtual only primary care clinician. Work with both patients and their families Optimize Care Delivery Strategy and Best Practices Collaborate with the full CloseKnit team to improve provider processes in order to optimize patient outcomes. Work with the team to apply continuous improvement processes to improve quality and patient experiences. Requirements QUALIFICATIONS: Education: Bachelor’s and master’s degree in nursing. Licenses/Certifications: Licensure in the State of Massachusetts and willing to obtain additional licenses. Board-certified by either ANCC or AANP. Prescriptive authorization including DEA in Massachusetts. Experience: Minimum 4 years’ experience providing primary care as a Family Nurse Practitioner.
