The purpose of this position is to provide quality social work services to the patients of Bridges to Health by assessing patient and family needs, obtaining appropriate community resources, and working closely with practitioners to maintain quality care.
Duties and Responsibilities:
- Identify those eligible for services (Intakes and Recertifications)Assist and prioritize social needs for patients and families (Identify social supports)
- Help patients cope with and solve everyday problems (referrals to other agencies, assist with locating and applying for housing, rental aid, utility assistance, etc.)
- Collaborate with team to assess client needs for treatment plan
- Advocate and develop plans to improve patient’ well-being
- Research and update community resource manual and asset map
- Refer patients to dental resources and document those assisted
- Pursue potential funding sources by locating and applying for grants
- Develop fundraising activities and strategies
- Update and print patient handbooks
- Communicate and obtain information from staff/volunteers for TORT
- Submit TORT applications as needed ensuring FTCA compliance
- Search NPDB and AMA to perform background checks
- Search Medicaid and HIP database to ensure patient compliance with other insurances
- Assist staff and volunteers as appropriate
- Answer telephone, schedule appointments, communicate with public and potential patients
- Pick up donations and supplies when needed
- Collect data and create quarterly reports
- Attend board meetings
- Public speaking functions on behalf of Bridges
- Provide tours and Bridges to Health background to volunteers/public/potential donors
- Check to ensure all required patient labs or testing is present for appointments
- Assist with rooming patients and taking vitals when needed
- Assist in medication room when needed
- Assist with MAP medication applications as needed
- Assist with making referrals to specialty physicians when needed
- Coordinate with Overdose Lifeline for Narcan and performs trainings and Narcan distribution
General Qualifications:
- Completed FAFSA on file demonstrating financial need; and
- Maintain an aid-eligible Satisfactory Academic Progress (SAP) status; and
- Maintain a minimum of 1 credit hour of enrollment in eligible courses; OR
- Be enrolled in a minimum of 1 credit hour in the upcoming term
