JOB OVERVIEW

We are seeking a compassionate and dedicated PSW specializing in Rehabilitation and BSO to join our healthcare team.

SKILLS

  • Graduate of an approved Personal Support Worker program in Ontario
  • Knowledge of and experience in the long term care environment and geriatric care
  • Restorative care education training an asset
  • Training in the Erie St. Clair BSO Champion Core Competencies including TBC on the Run, P.I.E.C.E.S., Gentle Persuasive Approach, U-First!, Person Centered Care EModule, BSO DOS, BSO Foundation & Toolkit and Dementiability preferred
  • Nursing rehabilitation and restorative care training, RCET (Restorative Care Education Training) an asset
  • Experience and education with quality improvement processes preferred
  • Knowledge of the principles and applications of customer service
  • Computer skills (email, Word, Excel and documentation software)
  • Excellent written and verbal communication skills
  • Demonstrated knowledge and skill working with individuals with older adults presenting with or at risk for responsive behaviors that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
  • Demonstrated effectiveness as a team member through collaboration, respect and effective communication skills in interacting with clients, family members and other members of inter-professional care teams
  • Understand and apply cultural competency
  • Demonstrated ability to effectively and efficiently handle demanding workload
  • Ability to conform to shifting priorities, demands, timelines through problem solving capabilities
  • Ability to adapt positively and productively to changes in the work environment

Responsibilities
  • Maintains a Resident centered approach in assessment, planning, provision and evaluation of resident care
  • Participates in conducting behavioural assessments and collaborates with the interdisciplinary team
  • Utilizes tracking tools and team consults to identify potential behaviour expressions
  • Coaches other long term care team members in behaviour expression techniques as required
  • Implements strategies, follows care plan and assessment goals
  • Executes and supports BSO centered bathing and care provision
  • 1:1 resident specific or small group programming
  • Utilizes therapies to assist residents with relaxation, coping with pain, anxiety and acute conditions
  • Implements a wide range of rehabilitation care programs included but not limited to eating or swallowing, walking program, range of motion, bed mobility, brace or splint assistance, transfers, dressing and grooming, communication and toileting and bladder continence
  • Assists with the implementation of care plan recommendations
