Qualicare offers home care services that focus on providing the best quality of life for our clients and peace of mind for their families across the Lower Mainland of BC. Overview: This role combines business development with client intake coordination. You’ll cultivate relationships with hospitals, physicians, and community organizations to generate referrals, while also performing client consultations and assessments to ensure a smooth transition from hospital to home care. Responsibilities: Build and nurture relationships with discharge planners, physicians, and community organizations to grow Qualicare’s client base. Educate referral sources about our services through presentations, events, and meetings. Conduct clinical assessments for prospective clients, reviewing medical records and recommending appropriate home-care services. Guide families through care options, answer questions, and coordinate service start dates. Collaborate with the care team to ensure timely admissions and seamless service delivery. Maintain accurate records in CRM systems and report on referral and sales targets, ensuring compliance with privacy and healthcare regulations. Qualifications: Existing relationships in hospitals or community health networks. Strong communication skills; ability to interpret clinical information and explain care plans to clients and families. Valid driver’s license and willingness to travel across the Lower Mainland.
