Job Details Job Location: Hospice Savannah Inc. - SAVANNAH, GA 31406 SUMMARY: The CAPABLE/GAP Registered Nurse (RN) will collaborate with older adults to deliver nursing components of the CAPABLE program after completion of required CAPABLE training through Johns Hopkins University School of Nursing. CAPABLE is a home-based program to address functional opportunities and promote independence and tenure in the community with older adults. The RN will work with CAPABLE participants to understand their goals, assess and suggest strategies in people’s homes who have volunteered to participate in CAPABLE. The CAPABLE program consists of up to four RN home visits over 4-5 months. Clinicians meet with participants in their home unless otherwise determined. The RN uses training and motivational interviewing techniques to conduct a comprehensive interview to identify and prioritize issues that matter to the participant. The RN documents the participants’ own goals in up to 3 Action Plans. The RN interacts and communicates with the CAPABLE Occupational Therapist (OT) and other team members as needed to ensure close collaboration throughout and after the program. As a part of the GAP program, the RN will be responsible for completing the nursing assessment for the Home and Community Based waiver for the State of Georgia. The RN would also assist with helping the participant with coordinating needed services. The GAP program is a home- based transitional program that helps chronically- ill older adults obtain Long Term Care Medicaid funded services, while supporting their immediate needs. Qualifications QUALIFICATIONS: Must be a Licensed Registered Nurse by the state in which she/he is practicing. Ability to work from a collaborative, client-directed model. Willingness and ability to implement a protocol and balance clinical judgment Computer proficiency in Microsoft Office to include Word and Outlook, and database knowledge required. Preferred Qualifications: include a bachelor’s degree in nursing and preferred experience in homecare. PHYSICAL DEMANDS: Position requires moderate visual concentration for patient charting/documentation. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The RN requires the mobility to ambulate or lift a patient with or without the assistance of devices more than 75 pounds using proper body mechanics. WORK ENVIRONMENT: A full-time position works a 30-40 hour work week with occasional requests to work additional hours as needed. At times, due to patient census and acuity, this position may be required to remain on site at Hospice House during meal breaks. The RN position may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). This position encounters stressful situations with patients and their families regarding end-of-life care. The RN must be able to accurately, timely and concisely document the patient/family encounter using a computer. The RN is required to perform patient visits utilizing their personal vehicle. Although each RN may be designated to a specific department/region in our service area, at times they may be required to serve patients outside of their department/specific region.
