The post holds many development opportunities and offers a chance to manage an autonomous caseload and plan care around patient centred goals
The successful candidate will work the hours over 5 days, Monday to Friday, and manage a caseload of cardiac rehabilitation patients within the community setting. The service is clinic based with occasional home visits You will also work on the ward providing stage 0- 1 of the commissioning pack pathway & in community exercise classes.

Two years post registration experience, 1 of which must be in recent CVD care
Understanding of the Commissioning guide for Cardiac Rehabilitation
