      Care Access Associate Reports To Care Access Clinical Team Leader Status Non-Exempt Shift Refer to Offer Letter Location Refer to Offer Letter                  POSITION SUMMARY:    Coordinates the referral aspects of a patient’s admission from initial inquiry through the transfer of referral to the clinical team.   ESSENTIAL DUTIES & RESPONSIBILITIES: * Coordinate referral calls from various referral sources, such as hospitals, nursing homes, assisted living facilities, and physician offices  * Communicate agency services to prospective patients, families, and other referral sources * Facilitate communication between and among the Palliative Care Team and Administrator, Hospice Divisional (clinical) directors, Response Center Team, and Referral sources * Confirm payer information and facilitate completion of necessary consent documentation to start service  * Enter patient data into an electronic medical record system * Abide by all agency privacy, confidentiality, and HIPAA policies * Coordinate all electronic referrals and record retrieval when necessary with Four Seasons teams * Answer, direct and track all incoming inquiries about Four Seasons services * Perform other duties, as assigned KNOWLEDGE, SKILLS & ABILITIES:  * Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation * Knowledge of HIPAA privacy rules and regulations * Knowledge of electronic medical records systems * Intermediate data entry skills * Strong attention detail and accuracy * Excellent organization skills * Excellent interpersonal skills * Excellent written and oral communication skills * Ability to work under pressure to meet deadlines * Ability to work with minimal supervision * Ability to interact with various departments throughout the organization * Ability to work flexible hours, if needed * Ability to participate as a team player to coordinate projects * Ability to plan, prioritize and coordinate multiple projects   MINIMUM QUALIFICATIONS:               Education:        * High School Diploma/GED   Experience:    * 2-3 years of progressive referral/call center administrative experience within a health care, home care, and/or hospice environment preferred   Licensures & Certifications: (if applicable) * Valid driver’s license and proof of automobile insurance   SUMMARY OF PHYSICAL REQUIREMENTS:    * Walking, standing, sitting * Bending, reaching, twisting, turning * Climbing stairs/other   INTENT & FUNCTION:   All descriptions have been reviewed to ensure that only essential functions and basic duties have been included.
