Your opportunity

  • Northern and Western Sydney service regions
  • Monday to Friday, 8:30am–4:30pm or 9am–5pm – no weekends

Join our team in a permanent full-time role supporting clients across Northern and Western Sydney. You’ll lead the delivery of high-intensity, short-term care designed to enhance health outcomes, maintain independence, and delay the need for more intensive aged care services—empowering clients to live well at home.

With care and professionalism, you will:

  • Deliver personalised, person-centred case management
  • Conduct assessments and coordinate in-home care services and support plans
  • Monitor and manage care packages within budget to optimise outcomes
  • Build positive relationships with clients, families, health professionals, and other service providers
  • Ensure accurate documentation and compliance with aged care standards

What you’ll bring
You’re a compassionate, organised professional with a strong commitment to supporting older people. You build trusting relationships, work collaboratively, and have strong problem-solving skills.

You will also need:

  • University Health Qualification (e.g. Allied Health, Nursing) required
  • Diploma in Aged Care, Community Services, or equivalent may be considered
  • Previous experience in Home Care, Community, or Aged Care settings
  • Strong understanding of client-centred service delivery and care coordination
  • Intermediate computer skills (MS Office)
  • A valid driver’s licence is essential, with regular travel across service regions required.
  • Commitment to Anglicare’s values of integrity, justice, compassion, and excellence
  • Experience working under the Aged Care Quality Standards
