    We’re on the lookout for a warm and outgoing Care Practitioner to join the team at our new 52-bed home, Langtree, nestled within the Standish Care Village.
Responsibilities

ROLE OVERVIEW:

As a Care Practitioner, you will support the Senior Care Practitioner and Nurse in Charge in delivering the highest standards of person-centred care preserving the dignity, respect and wellbeing of our residents at all times.
You will support and enable residents to maintain their skills and personal interests implementing the Household Model of Care at all times whilst maintaining your skills and undertaking such training and development as required.
Experience working with advanced dementia is necessary.

ROLE SPECIFIC DUTIES:

  • Ensure that residents are at the heart of care delivery and their wishes and preferences are taken into consideration to enhance their wellbeing
  • To understand personal care planning and taking responsibility for all aspects of care where required
  • To contribute to the efficient running of the home at all times
  • To understand the Household Model of Care and ensure you are championing this at all times
  • To support residents to maintain their relationships and connections with the local community
  • To ensure the highest standard of individualised person-centred care and attention is given to all residents in the home whilst treating them with dignity, privacy and respect at all times
  • To support and assist residents with their personal care such as washing and dressing
  • To administer daily medicines in line with their GP instructions and liaising with the Nurse in Charge and/or Senior Care Practitioner once this has been completed
  • To undertake minor medical duties such as oral hygiene, application of Topical Creams, skin integrity checks, catheter care and end-of-life
  • To ensure along with the wider team that all the residents’ needs are being met for example their medical, physical, social and emotional needs
  • Be responsible for promoting and safeguarding the welfare of the residents we are supporting.
