We are seeking a dedicated and skilled Care Case Manager to join our dynamic team in the Health, Wellness, and Fitness industry. The ideal candidate will play a crucial role in assessing and managing the care needs of our clients, ensuring they receive comprehensive and coordinated services tailored to their health objectives. The Care Case Manager will work closely with clients, their families, and healthcare professionals to develop individualized care plans that promote health, longevity, and overall wellbeing.

REQUIREMENTS

Bachelor’s degree in Nursing, Social Work, or related field; Master’s degree preferred.
Previous experience in case management, patient advocacy, or a related role.
Strong understanding of healthcare systems and community resources.
Exceptional communication and interpersonal skills; ability to build rapport with clients and families.
Proficiency in case management software and Microsoft Office Suite.
Valid license in relevant field (e.g., RN, LCSW) is preferred.
Ability to handle sensitive information with discretion and maintain confidentiality.
The pay range for this role is:
20 - 25 USD per hour(H

Responsibilities

Conduct thorough assessments of clients’ needs and circumstances.
Develop and implement individualized care plans in collaboration with clients and their families.
Monitor clients’ progress and make necessary adjustments to care plans as required.
Collaborate with a variety of healthcare professionals, including doctors, nurses, and specialists, to coordinate care.
Provide education and resources to clients and their families to facilitate informed decision-making.
Advocate for clients’ needs with healthcare providers, insurance companies, and community resources.
Document all interactions, assessments, and care plan modifications accurately and in a timely manner.
