Are you an organised, compassionate professional who thrives on making a difference in the lives of others? Right at Home Brisbane West is seeking a Full-Time Care Coordinator (12 month contract with the possibility of ongoing) to join our dedicated home care team, supporting clients across aged care and disability services.

QUALIFICATIONS

Certificate III Individual Support/ Diploma of Community Services or equivalent.
2 years experience in the health industry, preferably in care management.

ABOUT THE ROLE

As a Care Coordinator, you’ll play a vital role in connecting our clients, their families, and our team of care professionals. Your focus will be on ensuring every client receives high-quality, person-centred support tailored to their individual needs.

KEY RESPONSIBILITIES

• Coordinate and schedule in-home care services
• Match clients with suitable support workers based on skills, preferences, and compatibility
• Maintain regular communication with clients, families, and care staff
• Monitor and update client care plans to reflect changing needs
• Ensure compliance with NDIS and Aged Care Quality Standards
• Deliver exceptional customer service and maintain accurate records
