As a Care Co-Ordinator, you’ll play a key role in supporting patients through every step of their care journey. Working across Amesbury and nearby communities, you’ll become a vital point of contact for patients and their carers, someone they can rely on for guidance, reassurance, and support.
You’ll work closely with GPs and other healthcare professionals within the Primary Care Network to identify and support patients who would benefit from extra coordination and care planning. By helping them navigate the healthcare system with ease, you’ll ensure their needs are met, their care is well joined-up, and their wellbeing is always the top priority.
